Penn Medicine’s OCD therapy center sought a way to expand the reach and impact of its evidence-based treatment programs. Penn partnered with SemperCon to design and develop a digital companion that could help deliver consistent, personalized care while reducing the burden on therapists. The result was the BOLD OCD App: an AI-powered platform built to guide patients through structured exposure therapy, track their progress, and support clinicians with actionable insights.
Cloud-connected, AI-powered interactions feeding secure, web-based dashboards now provide Penn Medicine’s therapists with continuous visibility into patient activity and symptom trends. Patients, in turn, benefit from a guided experience that keeps them engaged and supported throughout their therapeutic journey—even outside of scheduled sessions.
BUILDING A DIGITAL THERAPY PLATFORM THAT MIMICS AND EXTENDS THE CLINICAL EXPERIENCE
A significant part of OCD treatment relies on routine, behavioral exposure, and consistent follow-through—challenges that often go unmet in traditional therapy formats. SemperCon worked closely with Penn’s clinical leadership to design an experience that replicates the tone and structure of in-person therapy using conversational AI. The platform includes a Progressive Web App for patients and a browser-based dashboard for therapists, both backed by AWS cloud infrastructure for security, scalability, and compliance.
The BOLD OCD App uses a combination of cutting-edge AI tools—Whisper for voice transcription, ChatGPT for context-sensitive dialogue, and Eleven Labs for speech synthesis—to create a lifelike, supportive interaction loop. Patients speak naturally to the system, receive tailored guidance, and complete structured exercises based on established therapeutic methods. Meanwhile, therapists gain access to automatic session summaries and real-time symptom data, enabling more precise, personalized care.
SemperCon also employed a suite of open-source technologies including TensorFlow, PyTorch, Hugging Face, FastAPI, FAISS, and PostgreSQL to ensure the app could scale efficiently and adapt to ongoing clinical feedback.

THE RESULTS
SemperCon delivered the BOLD OCD App on time and on budget for a well-received pilot at Penn Medicine’s OCD therapy center. The application enabled a dramatic improvement in therapist efficiency—reducing the time spent per patient from four hours to one, effectively quadrupling capacity without sacrificing quality of care.
Patients reported high levels of satisfaction, noting the natural conversation flow and feeling of ongoing support. Therapists appreciated the clarity of session summaries and the ability to track progress between appointments. The AI’s ability to detect and redirect off-topic responses kept sessions focused and productive.
